At Consolidated Primary Care, the concierge medicine packages allow you and your family to receive the medical care you all need from the comfort of your home. The practice offers primary and acute care, chronic disease management, and medication refill prescriptions in one convenient place. 

What is included in concierge medicine?

Consolidated Primary Care has three concierge plans to choose from. Along with a one-time administration setup fee of $50, the practice offers individual and family packages:

• $50 per month for individual — comes with 3 visits a month
• $100 per month for a family of 4 — comes with 6 visits a month
• $125 per month for a family of 6 — comes with 6 visits a month

*All procedures, injections, labs, and special tests will require additional fees

Depending on your needs, Consolidated Primary Care can recommend the best package and create a personalized scheduling plan that fits your lifestyle. 

Your visits can be in-office or through telemedicine. With telemedicine, you’ll need to connect with your provider using a phone, laptop, or tablet. At each visit, they can answer any of your questions and guide you on the next steps. 

For example, you may need help monitoring an ongoing health condition like diabetes. Your provider can follow up with you during each visit to ensure your blood sugar levels aren’t too high and provide guidance for other health issues, like diabetic neuropathy, that might affect you.
